HAP Midwest MI Health Link was approved by the State of Michigan and the Centers for Medicare & Medicaid Services (CMS) to provide you services as part of the MI Health Link program.

MI Health Link is a program jointly run by the State of Michigan and the federal government to provide better health care for people who have both Medicare and Michigan Medicaid. Under this program, HAP Midwest MI Health Link members will receive both Medicare and Michigan Medicaid health care services.
Important Update
MI HEALTH LINK (MMP) CONTRACT UPDATE ANNOUNCEMENT
In an effort to remain compliant with Center for Medicare and Medicaid Services (CMS) and Michigan Department of Health and Human Services (MDHHS) provider contract language requirements, we have updated our MI Health Link (MMP) Amendment/Addendum to include the required CMS language and updates for 2017. A copy of your updated Amendment/Addendum will be sent to your mailing address for notice provisions. The updated MMP Amendment/Addendum will be posted on the HAP Midwest Health Plan website and shall constitute written notice as outlined in your provider agreement.

What is MI Health Link?

Who is eligible for MI Health Link?
MI Health Link is a health care option for Michigan adults, ages 21 or over, who are enrolled in both Medicare and Medicaid. The goal of MI Health Link is to provide seamless access to high quality care that reduces costs for those who are eligible. MI Health Link offers a broad range of medical and behavioral health MI Health Link services, pharmacy, home and community-based services and nursing home care, all in a single program designed to meet the individual needs of the enrollees.
Who is HAP Midwest Health Plan?
HAP Midwest Health Plan is a for-profit, licensed Health Maintenance Organization (HMO) based in southeastern Michigan, and is a wholly owned subsidiary of Health Alliance Plan (HAP). Licensed in 1998 and accredited by the National Committee for Quality Assurance (NCQA) since 2000, HAP Midwest Health Plan has approximately 8,000 Medicaid and Medicare members.

Effective for services performed on or after August 1, 2017, Federally Qualified Health Centers (FQHC), Rural Health Clinics (RHC), and Tribal Health Centers (THC) must use the ASC X12N 837 5010 institutional format when submitting electronic claims and the National Uniform Billing Code (NUBC) claim form for paper claims. Claims submitted after this date using the professional claim format (CMS-1500 and/or 837P) will be denied.
