Disease Management and FREE Health Programs
At Midwest Health Plan (MHP), we care about the health and happiness of our members. We have a variety of programs that will help you and your children stay healthy and happy.
Programs include:
Diabetes Control Network
If you have Diabetes (high blood sugar), this program is for you! This program will give you facts on how to take care of yourself if you have diabetes and will send you educational materials in the mail. To join this program, call 1-888-654-2200 or 1-313-586-6071. After joining, you will be sent information on what diabetes is, how to control your blood sugar, taking medications the right way, exercising, eating right, eye and foot care and other important information. A letter about the program is also sent to your doctor so he or she knows about your progress. If at any time you wish to stop getting the information, just call Midwest Health Plan.
Asthma Focus
If you have asthma we can send you materials that can help. These materials will discuss: triggers and symptoms, using medicine and inhalers,
and what an Asthma Action Plan is and how to use it. If you have asthma and would like to enroll in our Asthma Focus program, please call 313-586-6071

A letter about the program is also sent to your doctor so he or she knows about your progress. If at any time you wish to stop getting the information, just call Midwest Health Plan.
Assistance to Stop Smoking
“I Can Quit” is a FREE phone-based program to help you quit smoking. If you join, you will work one-on-one with a health coach to develop a quit plan. Your health coach will call you to make sure you’re staying on track. You will also get a free quit kit with tools to help you stop smoking! ROSEBUDĀ® Pregnancy Program
This is a free program for members who are pregnant. The ROSEBUD program is available to help members achieve a healthy pregnancy.
A nurse that focuses in high-risk pregnancy care will contact you by telephone to discuss your pregnancy and general health. The nurse will help determine if there are any risks for early delivery or other pregnancy risks, and provide education and support. The nurse is available to answer any questions you may have. Maternal Infant Health Program (MIHP)
The Maternal and Infant Health Program is for pregnant women and their baby up to 1 year of age. This program helps pregnant members and infants get the proper food, support, and transportation for all health services. It will also help you to understand the importance of getting prenatal care, well childcare, and shots when they are scheduled. If you need help to get these support services, please call the Health Management Department at 1-313-586-6071.
Services include:
- Prenatal teaching
- Childbirth education classes
- Nutritional support and education
- Help with personal problems that may complicate your pregnancy
- Newborn baby assessments
- Referrals to community resources and help in finding baby cribs, car seats, clothing, etc
- Help with transportation to pregnancy related appointments
- Support to stop smoking
Preventive Health Reminders
Midwest Health Plan sends out preventive health reminders to members that may be due for services. These reminders include:
- Child Vaccines
- Adolescent Vaccines
- Lead Testing
- Well Child Visits/Well Adolescent Visits
- Diabetes Care
- Mammogram and Cervical Cancer Screening
- Colorectal Cancer Screening
- Physicals
Preventive Health Guidelines
Preventive guidelines are included in the new member welcome packet with a current immunization schedule. The guidelines are also on the website under the Preventive Health section. If you would like to request a copy of the guidelines, please call 1-313-586-6071.
Health Fairs
MHP holds an annual health fair each year for Midwest Health Plan members, their families, and the community. In the past health fairs have included free information on various health topics. Free health screenings were available for members of Midwest Health Plan including: blood pressure screenings, body fat, hearing and vision screenings, and many more. The fairs also feature health speakers on various topics including: Asthma, Heart Health, Diabetes, Domestic Violence, Woman’s care, and Nutrition.
Member Newsletters
The newsletters include various health prevention and disease management information. The articles discuss the importance of certain health conditions and screenings that are important in preventing many diseases. The newsletters are also a way to let members know about important programs and services available.
New Mom Packets
All new mothers receive a new mom packet from Midwest Health Plan. In this packet, new mothers are encouraged to obtain timely postpartum care and will receive a gift card for having a post partum visit. Postpartum care must be completed 21 to 56 days after delivery to qualify for the gift card.
Also, in the packet, is the Edinburgh Postnatal Depression Screening tool. Members are encouraged to complete the form and return it to MHP for a gift card. All returned screens are scored according to the Edinburgh guidelines. If the score indicates a member is at risk for postpartum depression, a second screen is sent. If the score on the repeat screen still suggests she is at risk for postpartum depression, she is sent a booklet called Depression During and After Pregnancy and encouraged to contact her PCP for further evaluation. Results of both screenings are mailed to the PCP to assist with any follow up care that may be needed.
Health Risk Assessment
Midwest Health Plan sends Health Risk Appraisals (HRA) to all new members in an effort to learn more about the current health status of our members. The member has a chance to fill out the survey and return it for a $5 gift card. The member can also request information on various diseases and health topics.

Member Incentive Programs:
Preventive health reminders are sent to members due for services. An incentive form is included to have the member’s doctor sign and return to MHP, verifying that the service was completed. Please call the Health Management Department at 313-586-6071 with any questions.

Aged, Blind and Disabled Population
MHP also has a member incentive program for our aged, blind and disabled members. Annually, all members that are in the ABAD category are sent letters asking them to see their PCP for an office visit. If the member sees their PCP and returns the form, they are sent a gift card.
