Midwest Health AKM is a medical service provider with the WayneCounty Health
Choice Program. Midwest Health AKM is an affiliate of Midwest Health Plan, a
state licensed Health Plan serving over 70,000 residents of southeastern
Michigan that attained NCQA (National Committee for Quality Assurance)
“excellent” accreditation. This accreditation is the good housekeeping seal of
approval for managed care plans. This Accreditation demonstrates the emphasis
Midwest Health AKM and its affiliates place on quality care. In accord with the
Wayne County Health Choice Program benefits, Midwest Health AKM provides a
continuum of quality health care services to enrolled subscribers and their

Midwest Health AKM offers an extensive network of providers including board
certified primary care and specialty physicians in Wayne and surrounding
counties. All of our providers have gone through a rigorous credentialing
process that meets Michigan ’s and National Regulatory standards. In addition,
we are affiliated with area hospitals including all seven hospitals of the
Detroit Medical Center , Oakwood Medical Center , St John Main and Garden City
hospital. We are also affiliated with over 1000 pharmacies in the tri-county
area including Rite Aid, Walgreen, CVS, Kroger, Walmart, Knight, Costco and
more.
Midwest Health AKM offer free disease management programs for persons with
asthma, diabetes, depression, high cholesterol and high blood pressure.
Midwest Health AKM also participates in several community health fairs
throughout the year. These health fairs discuss topics such as well women care,
immunizations, stress and depression, diabetes, well child care and other
important topics.
