Do you need health insurance, but have not been able to afford it? If you have a limited income, you may be eligible for our Healthy Michigan Plan. With the Healthy Michigan Plan you get the coverage you want and the support you need. We’ll help guide you along the way. Plus, we’re local and a part of this community. You are eligible if you are a Michigan resident between the ages of 19-64 and make up to 133% of the Federal Poverty Level. That’s if you earn roughly $16,000 a year as an individual or $33,000 a year with a family of four and you are:
  • Not currently eligible or enrolled in Medicaid
  • Not currently eligible or enrolled in Medicare
  • Not pregnant when applying
  • Ambulatory patient services
    • Visits to your personal care physician (PCP), nurse practitioner, physician’s assistant, or specialist, Outpatient hospital visits, surgical centers, home health care, hospice, podiatry (foot) care
  • Emergency care services
  • Hospitalization
  • Maternity and newborn care
  • Mental health and substance use disorder treatment services
  • Prescription drugs
  • Rehabilitative and habilitative services and devices
  • Physical therapy, occupational therapy, speech therapy, chiropractic, prosthetics, orthotics, medical equipment, medical supplies
  • Laboratory and X-ray services
  • Preventive and wellness services and chronic disease management
  • Pediatric services (including dental and vision care)
Other services including non-emergency care transportation, family planning services, programs to help quit smoking and skilled nursing facility.
